RUQ pain, pathophys. A 50-year-old, malnourished, hungry homeless man is admitted for evaluation of jaundice, ascites, and tenderness in the right upper abdomen. His temperature is 38 C (100.4 F). Physical examination reveals mild hepatomegaly and splenomegaly, as well as generalized muscle wasting. He has never had any tattoos and he is not sexually active. Nor is he a IV drug addict. No travel history. Ultrasound examination confirms the presence of an enlarged liver and a small amount of ascitic fluid. Laboratory studies show: (look carefully).
